Charts lose reference in docx

The doc seems to be creating properly but when I click on the chart and then click edit data. The data is there but when I return to the document All I get is #REF.

Have a missed something when creating the document to realign all the bits and pieces once the document is complete.

I am also having issue with charts that are being imported from docx sources. when I go to edit the data in excel the data values are set to blank.

Please help if you can.
